Celeste's Story
A story of perseverance

Celeste is a vibrant, joyful personality with a love of socialising, church and arts and craft.
Celeste has complex care needs with intellectual disability, Parkinson's disease, cerebral atrophy, cellulitis and edema.
Celeste originally only had a small NDIS plan, enough to give her assistance to access the community. She bounced from one retirement village to another with them unable to provide her with the level of care she required. Being under the age of 65, Celeste was able to access the NDIS.
Celeste eventually ended up in hospital. Her Support team had no plan to ever get her out of hospital and she was effectively put in the too hard basket.
After 9 months stuck in the same hospital bed, Celeste was disheartened and losing hope fast.
Aim to Enhance was connected to Celeste through the Hospital Social Worker team who were making one last effort to try and find her some help. Aim to Enhance recognised the seriousness of the situation and met with the Family, Social work team and Doctors to come up with a plan of action.
Within a month of being introduced to Celeste, Aim to Enhance had moved her to a lovely 3 bedroom assisted living home in Traralgon with 24/7 support.
Celeste is now living a fulfilling life, enjoying home cooked meals, outings to her favourite shops every week and church every Sunday.
Celeste’s life is now in complete contrast to her isolated existence in the Hospital. She is now able to build her capacity everyday with the Aim to Enhance care team supporting her every step of the way.
